I need this as well for zoom levels over 16... should can I build this using Visual Studio?

I think passing the -DFAT_TILE_INDEX to this line https://github.com/systemed/tilemaker/blob/master/Dockerfile#L31 & building the image (as per docs here) should solve your issue @tdav
