Default keymaps are not working

I think 4121726c37030f860cf77f3971f73543af444268 breaks the default keymaps advertised in the README.md since there is no default value for the keymap option.

It also doesn't handle insert mode maps.

I can probably submit a PR to fix it soon if you don't have time to do it :)

@madx is it just keymaps are broken for you, or highlighting too? I'm not getting anything after successful installation -- wondering if the cause is the issue you identify here or just I need to adjust the installation order in my init.lua.

@sjwo iirc highlighting was working fine when I used it. I don't anymore.
