Document use of library hostname in URL

Library URLs can now include a hostname (3.8)

Spoke with @EmmEff about this, and there is just no natural place to insert it. The quick-start, remote and cloud library docs deserve a re-write to make them more cohesive, and to provide a place to add in detail like this.
