First basic version of "Monitor-OS" documentation

This issue consits of two tasks:

  1. Explain the "Monitor-OS" as such and how to use it

  2. Write documentation about how to make and publish new OS functions (Monitor functions) in asm, VASM and C Also include the nitty-gritty details of parameter conversion from C to ASM and back as it happens in the VBCC monitor lib. (This is Inspired by https://github.com/sy2002/QNICE-FPGA/issues/105#issuecomment-687870293, where I saw, that it is necessary to document that somewhere.)

doc/monitor-os.md is a stub that already constains some thoughts. It needs to be linked from the index part in doc/README.md as well as from other places.
