Enhance Releases and Changelog

Describe the feature

Currently, when there is a release it's not showing the changelog on the releases section of the repo. It should al least show a link to the CHANGELOG.md file.

Also. On the changelog itself, when there is a #XXXX reference to a PR or Issue, it's not linking it to the related item. So you have to find it by yourself with the number on the PRs or Issues.
