[RESTRUCTURE] Restructure documentation to reflect separation of concerns: package vs. application

After separating services into other projects, we need to reflect this change in the documentation.

Plan:

1. Each plugin will have a README.md in the repo
2. This README.md will be based on a template in the /plugins/template
3. We will copy this file to the docs build
