Resolves the sample dataset portion of #326. Additional formatting changes were also made.

Hey Kevin, let me know if you're still interested in doing stuff with PyHealth. Hate to say that we've changed a lot of the stuff underneath the hood regarding caching (there's a lot of little intricacies we're working through).

But, always happy to look for help in other directions.
