Updates to Report a connection problem form

Based on SW feedback - SW-4549

We decided to:

• [x] Always open the form in a new tab (#6185)
• [ ] (Nice to have) Pass information from the SW record into the Qualtrics form. Information to pass would be: title, first url of the resource they are trying to connect to (the first 856 url)

Steve found this guidance from Qualtrics to pass information. If we can do this, it would improve the user experience and give the eResource group better information.
