Add accepted list for names

To prevent the Scunthorpe problem for names, moderators can individually add certain user/nicknames to an "acceptable" list, which will then not be flagged as a filter violation. This would make it unnecessary to use more relaxed permissions, such as adding this member to the entire filter immunity list just for their name.
