[FEATURE] Generate table of contents for documents

Is your feature request related to a problem? Please describe. Currently all documents of KoP don't have a table of contents.

Describe the solution you'd like Since Github doesn't support [toc] for Markdown, we need to generate the table of contents via some tools like gd-md-toc. Take this PR for example.
