Add command to remove a blockdev from cache tier

Since it is always possible to remove or reconfigure the cache tier without loss of data, there should be a command that allows the user to do this, and does the appropriate thing -- either remove the cache entirely if it was the last blockdev inthe cache tier, or reinitialize the remaining devices to operate using the remaining set of cache devs.
