Document Windows install

The installation process on Windows seems to be a bit more than just gem install, due to Ruby setup. If we could make a guide with requirements and common steps and put it on the frontpage README, I am sure it would attract a greater user crowd.

Having said that, I have just experienced a seamless install on:

Windows 10
RubyInstaller, Ruby 2.3.3 (x64)
