Filling in the list of exceptions

1. Add a placeholder to the exception list entry field (inactive gray text with examples), including an example of an empty field.
2. Add the "Copy host and field name" option to the context menu of the history windows in the format in which they should be inserted into the list of exceptions, i.e. if the field name is empty, it should be copied as . For example - github.com:<empty>

It would be possible to add rule creation directly from the windows history list, but I'm afraid this will lead to unnecessary rules being created by inexperienced users.
