Add Telemetry Reporting

This patch introduces telemetry reporting across extension host and webviews.

🚧 Note: the package we use for reporting is not yet published. It needs to be added to the package.json for this to work. Once I cleaned up that package and have it published to NPM, I will circle back and add it.
