Add page that details criteria for listing on homepage

Apps need to integrate Blockstack Connect and meet several other criteria before we'll consider them for listing on the App.co homepage.

We should list these criteria explicitly and provide instructions on how to submit a request for listing to this GitHub repository, providing a template for this type of new issues.
