Contributing Guidelines for Blockstack/designs

There was already a contributing guideline here, but I added content to the existing markdown with how to track issues, contribute code, and creating pr's.

This is generally designed after the recently created contributing guidelines here. Which are in turn a refactored version of bootstraps contributing guidelines.
