Add adaptation_rep_header ACL

The new regex-based ACL tests adaptation response meta-headers aggregated in a master transaction. For example, it can test whether an ICAP REQMOD response followed by an eCAP RESPMOD response have at least one X-Infection-Found ICAP response header.

Also removed unused FmtConfig::hasAdaptToken and FmtConfig::hasIcapToken.
