Rough Draft of Form Login sample for Cookbook

@Buzzardo, here is a rough draft of a sample, along with some adjustments to the basic-auth recipe. Other than a few adjustments, I have left verbiage alone, but it would need a good pass or two from you afterwards.

We can discuss (after the holidays) and move forward from there.
