Issue deleting images from album
There's a problem with deleting images from the Gallery. I can select the delete option from the right-click menu and confirm the deletion; the image is removed from the server folder, but image still shows up in album, as if it were cached somewhere. There doesn't seem to be any file I can see that would be serving the image, and if I delete the contents of the /assets/components/gallery/cache folder, the image still displays in the album.
Update: I do receive an error message in the log, as follows:

I have attempted to clear the phpthumb cache, with no change.
This issue occurs in both MODx 2.2.0 and 2.2.2, and Gallery 1.4.0 and 1.5.0.
Update: This issue is not restricted to deletion of images, but also albums. I think I might have isolated the issue to a cache problem; in /core/cache/db/objects/galAlbum, there are cache files that are holding the information for the various albums. This information changes when albums are added or changed (I tested the album titles); but when the albums are deleted, the files don't change. The album row is deleted from the gallery_albums table, but not removed from the cache or from the Manager, until the cache is cleared. At that point the cache files are deleted as expected, and the Gallery component screen in the Manager is refreshed with the deleted item removed.
I've checked all file permissions and ownership and it's all as expected; like I mentioned, additions and changes to albums are noted in the cache by the editing, adding and deleting of cache files. The issue seems to relate to what isn't removed in the cache when an image or album is deleted.
Update: Also seems that, when I upload a 2nd image to the album, the Gallery overwrites the first image, without deleting the thumbnail for the first image. So both images use the 1st image as the thumbnail, and both images show the 2nd image as the full size version.
No explanation for this whatsoever...
I'm having the same problem deleting and re-ordering images in an album. There are no options when I right click, and no drag and drop. Not sure if this was available in earlier versions, but it seems that it would be a feature. I have it installed on MODx 2.3.1. Does anyone know of a fix for this?
