The medium.iso ECC was augmented by

dvdisaster --image ./medium.iso -mRS03 --create --no-bdr-defect-management

Hello,

12219392 sectors is the theoretical maximum size for a BD-R without defect management, and is indeed the value hardcoded in dvdisaster for this:

src/dvdisaster.h:#define BD_SL_SIZE_NODM      12219392

So you shouldn't end up with an image bigger than this. How much sectors is your medium.iso image before being augmented, so I can try to reproduce your issue? You may get this information this way, on your iso file before it is augmented by dvdisaster:

isosize -x medium.iso

EDIT: also, more information on how your image was created can also help (i.e. does it also have an UDF filesystem on it? etc.)
