bug: code changes are not being picked up

Describe the bug

Cody suggests changes and I apply them, but then the next cody message suggest I add the same changes that already exist. It fails to realize even after tagging the updated file that these changes have already been implemented.

Expected behavior

Pick up updated codebase as I continue coding.
