VS Code: Getting error while running command from command palette.

Describe the bug

Platform: MacOS

Steps to reproduce:

1. Have Cody plugin installed in your VS Code editor
2. Login to Cody with Pro user
3. Open source code file and right client to open context menu
4. Select "Command palette..." and choose "Cody Command: Ask Cody to Explain"

Observed behavior:

Getting error "Cannot read properties of undefined"

Expected behavior

Error should not occur and command should be executed correctly.
