Use a real markdown parser

Right now all of the Markdown parsing is done via regular expressions I copied from Gruber's Markdown.pl. A real parser would be a lot better. All of the ones I looked at when I was originally writing this didn't include indices, so it wouldn't work with Quesadilla since everything needs indices.

Last I looked at redcarpet (and sundown) they didn't support getting indices out. They should be revisited.
