pdf file name does not match post title error

I'm getting an intermittent odd issue, which is that the filename of the pdf (title-of-post.pdf) does not match the title of the post. If you hover over the pdf download link, the url is correct (www.myblog.com/title-of-post-is-correct.pdf) but when you actually download the pdf, the name of the file is incorrect. This does not effect all posts.

NB I'm not saving pdfs to media library. This is not caused by browser cache as have tested on a variety of devices and browsers. My settings only offer pdf, no other formats.

UPDATE: After more testing it appears I am only encountering this with two posts, which are in the same category, but not connected in any other way. Even after changing the title of 'post A' which seems to be the problem, 'post B' (which does not have the correct title in the pdf filename) still picks up the title of post A - the changed title in this case. I've left this error note up here in case others might encounter this weirdness.

Intermittent issues are such fun ...

The filename is "post_name (slug)".pdf, so please check if the slug is different from the title. I will change this and use sanitize_title instead.

yes its the post slug (that basically is the title) that is behaving like this. Sorry, I used the wrong term.
