Menus overflow on datasets with many constraints

per discussion, we agreed to punt on restoring the health_indicators as the dataset is too far from the new back end model. punting this once new api-backend rolls to prod unless there are any others folks know about.

There aren't any other datasets with more than two constraints, which fits comfortably with the current menu system. I think that we're OK punting on this one.
