Accessibility Features/Services at location

We have had requests from our Accessibility stakeholders to note what accessibility features and/or services are available at specific locations. e.g. wheelchair access, drive-through, interpreter services, etc.

Great point! @bionicscott I'll draft a PR for this, and would love your help enumerating a "value set" for accessibility features/services that we can use to get folks started.

For "drive-thru vs indoor clinic", http://hl7.org/fhir/location-definitions.html#Location.physicalType is a good property to use (i.e., we might not lump this property into a generic "features" extension)
