Command needs confirmation due to * rule, config not updating

I am getting this on both Pycharm and VSCode extensions. The impact is I can't auto run shell commands.

As per image

I have removed all 'Ask user' and 'Deny' rules as per the suggestion, from the configuration page.

I have also tried different LLM models, and tried local and global shell tool, and removing it and re-configuring. Nothing fixes it.

Happy to make the fix if you can point me to the location in code.
