Extra labels showing up under radio buttons on first page revisit

I have created a radio button form on the page. Upon going to another page and returning to the radio button page, there are labels that appear below the radio buttons with the same name as the text displayed on the buttons. This only happens when I revisit the page the first time - on subsequent revisits, no extra labels are shown.

Any insight on how to solve this would be terrific. Thanks!
