Trade screen title changes

As I requested for custom token trade, it will required to change the way of display the title of Trade screen.. It must to become SYMBOL/ETH as the price should be appears (on the top center of order book?)

Note: the icon on custom trading shouldn't appears if the token isn't have any icon on etherscan.org otherwise you should to fetch it from there..
