I see that you have done a lot of inline HTML styling. I guess it is better to create a separate CSS file for the styling of the page.

I have completed the styling the user interface and made it responsive. I have also added a typewriter effect to the title before the form. I completed testing the UI by deploying it on google app script. I have attached the UI below:

@Kanchan253 your UI is good too (please increase the size of header, and width of the box), received from @jayantsingh-22 first. But let us keep both, please create a PR with your file named as index1.html and in the file code.gs link index1.html as well, keep one of the two commented and write a proper comment to ask to choose one of the UI, to make developer understand. And don't forget to append your name in the Contributors List.
