Add Account - Revamp Ledger connect screen

Overview

We want to revamp the Ledger connection flow to look and feel better - see mock.

Note, this is decoupled from the Rabby-like accounts selection flow (i.e https://github.com/shapeshift/web/issues/6673) which is to be tackled last - do not implement the account selection CTA and step yet as part of this issue.
