[Bug]NCCL error if enable the cuda graph

Describe the bug
If I don't disable cuda graph, I will get the error shown in the picture when the cuda graph is being inited. If i use the official docker image, i will not get the error. The only difference of the environment with the docker is the sglang by observing the output of python3 -m sglang.check_env. I install sglang via pip and i have observed the sglang of docker image is installed from local.
Reproduction
python3 -m sglang.launch_server --model-path deepseekr1 --tp 16 --dist-init-addr 29.111.44.27:20000 --nnodes 2 --node-rank 0 --trust-remote-code --host 0.0.0.0 --port 8000
will reproduct the error above.
add the option --disable-cuda-graph will run well

I'm on the most recent version for CUDA 11.8 (nvidia-nccl-cu11 2.21.5) and get the same error when trying to run Llama-4.
