Doctor can not create Analysis Requests for patients

Steps to reproduce

1- Log in as a Doctor 2- Go to a Patient and try to create an Analysis Request for this patient 3- Insufficient Privileges

Current behavior

Doctor does not have privileges to create an Analysis Request for a Patient

Expected behavior

Doctors should be able to create Analysis Requests for patients, but I am not completly sure about how... So far a doctor can see all Patients, not sure if this is ok...

The Doctor should only be able to see the Patients from the same Client to which he/she is assigned. A Doctor can provide services for more than one client. Hence, one should be able to assign a Doctor to 1 or more Clients (atm, you can have Doctors w/o Client assigned).

Doctor should be able to create ARs and Clinical Cases, but the list of Clients (and therefore, Patients) must be limited to those the Doctor is assigned to.
