_falloff removed

the original VHLT has the HLRAD_ARG_MISC flag that when set, removes all falloff stuff. And when you remove all the #ifndefs you removed all the falloff stuff from rad.

please put them back, because frankly i have no idea how falloff works on a cleaned up repo like this as everything regarding falloff, including the global value (g_falloff), is gone.
