Merge upstream up to CQL5 support

We need to get CQL5 support. Let's merge all the changes up to https://github.com/apache/cassandra-gocql-driver/commit/63b6d7830710a0c6c5411c08bf16d38cb88d2a8a

Add https://github.com/apache/cassandra-gocql-driver/pull/1906 on top of it.
