There are some aspects of this which are connected to wide layouts. I think it's worth waiting to see what happens with https://github.com/WordPress/gutenberg/pull/29335 before solving these...

I rebased this and now I'm seeing a few visual issues, sorry. I'm not sure if these are related to the rebase:

I have fixed the issues above. Sometimes the toolbar ends up on the right, which is a bit surprising but it does work ok:

My understanding is that we are going to try this approach: https://github.com/WordPress/gutenberg/issues/61837 so I'll close this PR for now.
