### Description I noticed that the file path for the pull request template was incorrect, so I made the necessary adjustment. This pull request fixes the file path issue by changing it from CONTRIBUTIONS.md to CONTRIBUTING.md.

### Why The incorrect file path was causing confusion and potentially preventing contributors from accessing the pull request template. This change ensures that the correct template is used when creating a new pull request.

### Testing Done I have tested this change by creating a new pull request, and the correct template is now being applied without any issues.
