Get user inputs

Kia ora,

Is it possible to have a smart button ask the user for inputs - similar to the Dataverse Trigger When a Row Is Selected. This functionality would be spectacular.

The specific situation I am chasing is giving a user a date field, choice, integer, and two text fields. Ideally this information will be included with several other bits of data we can already get easuly - user and selected record details.
