Handle conflicting header IDs in link hygiene

The SemanticDB spec has three "Synthetic" headers https://github.com/scalameta/scalameta/blob/master/semanticdb/semanticdb3/semanticdb3.md#synthetic-1

Currently, mdoc doesn't append the -N suffix for conflicting names.
