Show an error image if a problem was encountered in parsing

An error image would help the user to understand where they went wrong when inserting an image into a page.

We already generate error images for unknown file types, so in theory this shouldn't be too hard to do - we just need to fiddle with the image syntax generation all over again (ugh) as it doesn't support detecting what the user did wrong just yet.
