SAK-44549 - Update references to morpheus-default/tool.css in tool markup

Formerly this "in tool" markup pointed at /library/skin/default/tool.css - the 404 was harmless unless the tool was not inlined into the portal markup (i.e. in an iframe).

@adrianfish Could you check to see if this will cause heartburn in trinity? It is a simple fix - just as easy to do post-trinity merge or pre-trinity merge. If it does not collide with anything in the trinity branch, it would be nice to put this in quickly and move along.
