Added Table of Contents and easy navigation to documentation.

This change adds a table of contents section to the bottom of every documentation page.

It also fixes an unclosed <ol> in one of the pages, and changes the contents of the <title> and <h1> of each page to include the "page" e.g. "3.1"

A minor change is that it removes trailing whitespace in the HTML.
