Is your feature request related to a problem? Please describe. Enforce the issue template using GitHub issue forms.

At last count, there are 464 issues that were automatically closed because they did not include an MRP or other required section. This is overloading my GitHub bot, and also my own bandwidth to manually type a reply to ask for the sections that were skipped over.

Describe the solution you'd like

1. Use GitHub forms to enforce the required structure.
2. Spring cleaning: manually delete the 464 malformed issues.

Describe alternatives you've considered

The alternative was to manually deal with it, but it now needs a proper solution.
