666 permission on index.php breaks site (internal server error)

Just saw a weird one when upgrading a site with this. I have actually seen it before many versions ago, but not since.

Not sure why the index.php file is being given that permission, but perhaps this module should chmod all files/files to match $config->chmodFile / $config->chmodDir settings?

Maybe, maybe not - maybe not good to overwrite how someone has manually set these up, but I still think the current situation is undesirable.

I don't manage this server, not sure if there is maybe something unusual that is causing this, but as I mentioned it doesn't happen very often.
