https://axiomsso.herokuapp.com/OAuth2RequestAccessToken.action the workbench hyperlink on "Examples Accessing Salesforce as User" seems to points to a wrong hostname. here mine: http://workbench/login.php?sid=...&serverUrlPrefix=https://test-dev-ed.my.salesforce.com Identity hyperlink works perfectly.
