Item duplication and frame blocks corruption by stone

The server had crashed, upon coming back I was greeted by this sight:

Running FTB Infinity 1.10.1

It seems that the ME chest has lost its drive, it's floating outside of it to the right as you see. Underneath the stream of ender pearls is a world anchor. With a full hopper of ender pearls.

This isn't the first time it happened. However the chunk border is between the redstone block and chicken chunks loader. And as you can see, the frames in that chunk turned into stone, with sometimes stone on top of where used to be frame blocks. I can't seem to reproduce this on demand, but yet it happened 2 times now.

The tries always included:

-1 Chicken chunk loader (3 chunk range) -1 Chicken chunk loader (3 chunk range) & World anchor in different next to each other chunks

I'm thinking the CCL is the culprit. Going to test tonight with just the world anchor.

This time it happened with MFR chunk loader.
