range: mandatory has no effect

Range-items always have 50% as default value. This results in the effect, that this item can't be set mandatory.

When changing this, we should offer the requested feature to make the knob appear on first click.

• [ ] don't show knob till first interaction
• [ ] default value should be out of the defined range or NA (in default optional usecase)
