Prep for 0.1.4 release

I bumped the version, tweaked the NEWS, and updated a URL that had changed; the rest is whitespace cleanup thanks to VS Code.

Locally, R CMD check --as-cran is good with this, and there aren't strong reverse dependencies, so this should be ok to submit.

I went through the open PRs and merged in a few bugfixes that were lingering. check passes locally. I'd like to get this out to resolve the dbplyr issue, and also so I can start doing some more significant changes.

@colearendt are you 👍 for submitting to CRAN? Alternatively, I can keep merging changes in and we can release in a couple weeks. (On second thought, maybe that's better?)
