Add option for gist title.

I regularly search my gists and rely on helpful names to find things. A name option for the gist to be posted would be useful. It would stop me from freaking that I am polluting my gist namespace with randomly generated filenames. That's value-add right there.

I am would be happy to make a pull with this at some stage.
