[vim] visual mode is unreliable and on keypress enters insert mode instead

I have vim navigation enabled, however 'enter visual mode' is unreliable for me.

When I hit 'v' to enter visual mode, more than half the times it'll instead enter insert mode.

When I press the escape key, I briefly see the visual mode highlight but it doesn't stay.
